Probably you will be asked to submit an affidavit if you get interview call. You must talk to Imran Farid sir. You can find his number in UPSC telephone directory.

hi shefali just keep calling they will pick up , but basically you have 2 options : - Best is if you are in delhi then go to upsc office or ask some frnd to go there and talk to them personally, they will require an affidavit from your side for the same. - or wait generally after mains they check all the documents and if there is any discrepancy the mail and ask for required documents withing 15 days.

I made a similar mistake in 2013. They told me to write an application and bring it with a photocopy. They took the application and put an official stamp on the photocopy which I was to keep. It will sought out any problem in future while interviews or while writing mains in examination centres. Always keep that stamped copy with yourself. and this is a very minor problem so do not worry.

Shefali, shoot a mail to UPSC asap and also talk to Farid Sir tomorrow. I'm sure he will help you out.
