Okay guys : I am creating a consolidated FAQ so that aspirants can ask their questions here, and get mental peace. Here is what I've gathered so far, please correct me wherever I am wrong.
Q) What should I mail in before my interview?
A) As far as I know, only the attestation form needs to be mailed in before the interview.
Q) Attestation form: By when to submit, to whom to submit, how to submit
Ans) It should be submitted to UPSC not DoPT. It should be submitted as soon as possible once your eSummon letter becomes available. Preferably before 10 days.
You need to fill out this form, but don't sign nor paste photos, then get page 6 attested by a gazetted officer and then take SIX copies. You need to sign on the original and copies and attest a photo to every place.
Q) What should be carried to the interview?
A) You should carry a copy of the Hallticket, Questionaire, class X to degree Certificates, caste certificate, disability certificate, other age relaxation certificate and revised preferences sheet and also ample number of photographs [10-20]
Q) What should be carried to the medical?
A) Go with an empty stomach, hallticket, copy of the latest prescription, and ample photographs.
Am I missing something?? Please correct me wherever I am wrong. Please feel free to ask more questions in this thread.

Also is it only 6 copies of attestation form that we are supposed to send or anything else. Also it is to be sent to ASI desk officer, DoPT,Ministry of personnel but u mentioned it should b sent to UPSC?
Also 12th point in instruction says " pls acknowledge the receipt of this letter immediately indicating your roll number. If you are an out station candidate then pls state the address at DELHi Where you intend to stay" what does this mean??
I too don't know how to "acknowledge the receipt of this letter". Can someone interviewed candidates throw gyaan??
